Description: The main goal of the D-CARE project is to enhance skills and develop smart solutions that strengthen and integrate regional social and healthcare systems across 9 Danube regions. The project promotes e-care and e-health services for older adults with chronic illnesses and cognitive impairments.
Project activities are organized around three pillars:
- Design of networked innovative learning environments responsible for developing new learning tools, content, and training programs to improve skills needed by project regions for providing new integrated care services (innovative smart care models).
- Implementation of smart care pilot projects to research, select, optimize, test, and validate innovative smart care services for older adults with chronic illnesses or cognitive impairments.
- Establishment of a Transnational Learning Policy Center to facilitate knowledge creation and dissemination, ensuring that policymakers can access and use project results and insights.
